Welcome

Modern health care systems must achieve three challenging goals: improved quality of care, improved population health and improved efficiency. These goals require the integration of services to meet needs, to prevent illness and to reduce duplication and waste. 

This conference will bring together researchers, clinicians and managers from around the world who are engaged in the radical redesign of services. They will share experience and the latest evidence about patient and population needs, patient-centered service design, and the information and communications technology needed to make integrated services a reality.

• Call for Paper Deadline: 18 July 2014.
  Abstracts of good practice, projects, development of policy and research and theory in integrated care are welcome. All accepted abstracts will have the opportunity to present a paper or poster at the congress.
